Service Coordination Unit
The Service Coordination Unit provides
services to anyone receiving mental health services. Service Coordinators help
individuals access mental health programs and services such as housing,
education, advocacy, vocational training, medical and legal needs.
A liaison is maintained with St. Clair Hospital,
and other local hospitals to coordinate discharge and follow-up services.

In addition to the services above, Chartiers offers specialized service coordination to address the needs of the individual. Services may
include:
Blended Service Coordination
Service Coordinators work with a treatment team to help individuals identify
strengths and goals which will enhance their quality of life. Service Coordinators assist them in achieving these identified goals. Services are
available 24 hours per day for individuals who require intense and immediate
services.
Alternatives Benefiting Consumers (ABC)
The Service Coordination Unit provides funding to individuals with
mental illness and their families for special needs in behavioral
management, therapeutic instruction and tutoring, and adaptive equipment.
Funding is limited.
Emergency Phone Consultation
Staff offer supportive counseling, diversion, and referral
services to individuals, their families, and community medical and
