Service Coordination Unit

photoThe Service Coordination Unit provides services to anyone receiving mental health services. Service Coordinators help individuals access mental health programs and services such as housing, education, advocacy, vocational training, medical and legal needs. A liaison is maintained with St. Clair Hospital, and other local hospitals to coordinate discharge and follow-up services.

Chartiers provides 24 hour emergency services for individuals and their families.

In addition to the services above, Chartiers offers specialized service coordination to address the needs of the individual. Services may include:

Blended Service Coordination
Service Coordinators work with a treatment team to help individuals identify strengths and goals which will enhance their quality of life. Service Coordinators assist them in achieving these identified goals. Services are available 24 hours per day for individuals who require intense and immediate services.

Hestia Project
In March 2012, Chartiers opened doors for the Hestia Project. The program is for individuals who are chronically homeless and have a disabling condition. Chartiers' Hestia Project will supply 62 single units and 2 family units. The units will be spread throughout Allegheny County with care taken to ensure they are near social services, treatment providers and transportation.

Once referred to Chartiers, the Service Coordination staff help the individual locate housing. The individual directly holds the lease to the property and Chartiers will use the HUD funding to pay a portion of the individuals rent. Operating as a Shelter Plus Care model, Chartiers staff will continue to provide services as needed and the individual is required to follow the terms of the lease. Chartiers HUD grant is part of the 8.3 million in federal funds received by Allegheny County to expand homeless programs.
