Case Management Services
Previously
known as the Base Service Unit, the Service Coordination Unit provides
case management services and crisis intervention to anyone 6 years of
age and older receiving mental health services. Case managers help
individuals access mental health programs and services such as housing,
education, advocacy, vocational training, medical and legal needs.
A liaison is maintained with Mayview State Hospital, St. Clair Hospital,
and other local hospitals to coordinate discharge and follow-up services.

In addition to the services above, Chartiers offers specialized case
management services to address the needs of the individual. Services may
include:
Intensive Case Management (ICM)
Case managers work with a treatment team to help individuals identify
strengths and goals which will enhance their quality of life. Case
managers assist them in achieving these identified goals. Services are
available 24 hours per day for individuals who require intense and immediate
services.
Resource Coordination (RC)
This program focuses on services similar to the ICM, but with a population
whose behavioral health needs are somewhat less demanding. Resource Coordinators
help individuals establish goals and find resources in the community
to assist them in achieving their goals. A 24 hour emergency service
is available.
Alternatives Benefiting Clients (ABC)
The Service Coordination Unit provides funding to individuals with
mental illness and their families for special needs in behavioral
management, therapeutic instruction and tutoring, and adaptive equipment.
Funding is limited.
Emergency Phone Consultation
Staff offer supportive counseling, crisis intervention, and referral
services to individuals, their families, and community medical and
