Service Coordination Unit
The Service Coordination Unit provides services to anyone receiving mental health services. Service Coordinators help individuals access mental health programs and services such as housing, education, advocacy, vocational training, medical and legal needs. A liaison is maintained with St. Clair Hospital, and other local hospitals to coordinate discharge and follow-up services.
In addition to the services above, Chartiers offers specialized service coordination to address the needs of the individual. Services may include:
Blended Service Coordination
Once referred to Chartiers, the Service Coordination staff help the individual locate housing. The individual directly holds the lease to the property and Chartiers will use the HUD funding to pay a portion of the individuals rent. Operating as a Shelter Plus Care model, Chartiers staff will continue to provide services as needed and the individual is required to follow the terms of the lease. Chartiers HUD grant is part of the 8.3 million in federal funds received by Allegheny County to expand homeless programs.
