Mobile Psychiatric Rehabilitation Worker

As a Mobile Psychiatric Rehabilitation Worker with Chartiers Center, you will work in the home and community with our program participants to help them to live full and productive lives. Job Duties include: 

  • Meet regularly with a caseload of clients in their home or community to teach them needed skills such as independent living skills, social skills, and job search skills, and help connect them with supports such as needed living resources and social opportunities 

  • Maintain a weekly schedule where the majority of your work time is spent in face-to-face client meetings 

  • Thoroughly document client meetings to reflect work completed and interventions delivered, as well as client’s response to service and any client concerns

            As a Licensed Practical Nurse with Chartiers Center, you are critical members of a multidisciplinary team and are responsible for providing direct skilled care under guidance of the Director of Residential Treatment Program.

            Responsibilities include the following: the plan of care, administering medication, maintaining current health profiles through documentation, completing medical aspects of treatment and working collaboratively with the members of the team to help our clients achieve their recovery goals.

            Shifts include Daylight, evenings, and weekends.

                    As a Therapist with Chartiers Center, you would be committed to providing compassionate supports and therapy to people facing mental health challenges. Job duties as a therapist include:

                    • Complete required evaluation and assessment for treatment of consumers
                    • Complete all required documentation to include, but not limited to managed care information, initial consumer assessments, service plans, service plan reviews, progress notes, correspondence, transfer summaries and terminations summaries.
                    • Plan and coordinate treatment for consumers through participation in treatment team meetings, interdisciplinary team meetings, psychiatric staffing, and individual supervision.
                    • Develop and implement individualized treatment plans for an assigned consumers utilizing goal-setting techniques.
                    • Maintain ongoing contact with Service Coordinators and other Agency program staff to ensure consumer accessibility of community/supportive services and continuity of care.
