Chartiers Center provides person-centered services that help individuals build independence, develop life skills, and reach their personal goals. Together, these mobile programs offer flexible, community-based support that empowers people to thrive where they feel most comfortable.

BLENDED SERVICE COORDINATION

Blended Service Coordination Services (BSC) is designed to walk beside you as you work toward your goals. Whether you’re seeking housing, mental health support, benefits assistance, or help managing life’s challenges, your Blended Service Coordinator is here to help you navigate systems and connect to the resources that make a difference. We take the time to understand what matters most to you and create a plan that reflects your strengths, needs, and vision for your future.
We are committed to honoring your voice, respecting your experiences, and building a partnership based on trust, hope, and recovery. BSC is not therapy; it’s support that helps you move forward. Your Blended Service Coordinator will meet with you where you feel most comfortable and work with you to remove barriers, problem-solve challenges, and celebrate your progress.

Literally! We can meet in your home, at a provider’s office, or in a community setting like a library, Point State Park, or a local coffee shop. Wherever you feel most comfortable!

Connect you to needed services

Blended Service Coordinators can help find housing, connect you to therapy or primary care providers, assist with applying for benefits (such as SSI or SNAP), develop crisis plans, coordinate transportation services, advocate during important meetings, and support transitions from hospitals, shelters, or treatment facilities.

Build and/or reinforce your natural supports

If you want your family members, partners, or other natural supports involved, we welcome their participation. You always control who is involved in your care.

MOBILE PSYCHIATRIC REHABILITATION

Psychiatric Rehabilitation Services are founded on the firm belief that people can and do recover from mental illness. Psych Rehab providers define recovery as a process of change and growth that allows one to feel more successful and satisfied with their life and achieve a greater sense of meaning and purpose. Our Mobile Psych Rehab (MPR) staff assist program participants with identifying the life goals that are most important to them and then helping them to learn, enhance, and maintain the skills and supports they need to reach these goals and their full potential.
MPR staff can work with participants on building living skills such as budgeting, utilizing technology, preparing meals, cleaning and organizing their home, and using public transportation. They can teach the skills needed to find and maintain a job or volunteer position, or to pursue further education or training in a field of interest. They can support participants in building social skills and getting involved in their preferred social groups and recreational activities. And they can teach overall wellness skills like building a more structured daily routine, improving diet and exercise habits, and practicing coping skills to manage symptoms and stress.

One-on-One Services in Allegheny County

While site-based services provide a valuable social skills aspect, Mobile Psych Rehab services can provide participants with one-on-one customized support and the ability to learn and practice skills in natural settings where they most need to utilize them, such as the home.

JANUS

Janus is a recovery-oriented, behavioral health permanent support housing program that helps individuals with persistent mental health diagnoses live independently in the community.
Janus is a recovery-oriented, behavioral health permanent support housing program that helps people with persistent mental health diagnoses live independently in the community. This program will support individuals in their home and community, allowing them to engage with their recovery, wellness, and achieve their life goals. Committed to a collaborative effort to support people, Janus also provides individualized in-home housing support services, such as service coordination and mobile psychiatric rehabilitation services. The program aims to build connection to social & peer networks, build daily life skills, job interviewing, self-advocacy and more.

Residents self-direct where they live

We want to make sure our consumers are comfortable in the community & neighborhood they choose to live in. Our consumers lease their home directly from landlords partnered with Chartiers Center.

Titrate services as needed

The goal of the program is help our consumers live
independently and achieve their goals. Recovery is not a linear journey. Our staff helps our consumers navigate the ups and downs and provide stability and effective skills to reduce the long-term impact of mental health challenges.

Connection to support services

Case Managers assist with accessing health & wellness services, vocational training, community network building and much more. Mobile Psych Rehab workers focus on skills building to determine strengths and areas of growth.

CREATIVE RECOVERY

Chariters Center’s Creative Recovery is a social rehabilitation program for adults facing mental health challenges. Participants learn social and vocational skills to rebuild self-esteem and confidence while fostering independence. Participants will discover many positive ways to express emotions and discuss them in a safe and encouraging environment.
Creative Recovery provides participants opportunities for socialization and vocational skills to help navigate and reintegrate into the workforce. Participants learn and practice these skills at our Mt. Lebanon headquarters through hands-on activities and discussions. Participants implement these skills out in the community by engaging in social, recreational and semi-professional activities.

ADDITIONAL SOCIAL REHABILITATION
IN STO-ROCKS!

Chartiers Center Connections (C3) is Chartiers Center’s social rehabilitation program in Mckees Rocks. C3 helps participants learn valuable social and vocational skills with discussion groups, and community activities. C3 is located at
827 Broadway Ave. Pittsburgh PA 15136.

No cost to be a program participant

This program is free to apply or participate. Residents must live in Allegheny County.  *Please note: Chartiers Center does not cover the costs of activities.*

Community building and peer support

Through sharing of valuable life experiences, our consumers come together to support one another, get help to stabilize their symptoms and build community rapport with individuals of all ages and backgrounds.
